If these hospitals permanently sent away every employee who turned up drunk on duty, they would be understaffed, so compromises have to be made.
Formalin is widely used as a disinfectant in hospitals and medics seem to have made a fatal error. Probably the bottle containing formaldehyde was wrongly labeled or a disgruntled hospital worker intentionally did it.
