1) The CP that went unremoved was attested to by the victim, who provided photo ID. They dealt with real people, not an algorithm. It took a call from the feebs.
2) The CP they just removed enmasse when elon came in was organized and shared using euphemistic hashtags as code words. I would not suggest you google these terms. So they certainly can use the same text checking algo, and did.
